How to get a health card for the first time ?
To get some health insurance rights and to make you refund your care of health, it is essential that you have in your possession a vital card. It is free and you can have access to it from 12 years old if your parents ask for it. The card is equipped with a chip that allows you to read the administrative information needed to cover your health care costs: identity of the insured, registration number, affiliation scheme, health insurance fund, etc

Ask for a “carte vitale” for the first time
The card is an essential element if you want to. When you go to a doctor, the treatment form is electronically transmitted to the Social Security: this transmission will allow you to have a quick reimbursement under 5 of your CPAM. By avoiding sending a paper health care form, you save a considerable amount of time on your reimbursement.
It also allows you to benefit from the third party payment system during your visits to the doctor.
How to order’get ?
As soon as you turn 16, without any action on your part, you will receive a letter from the CPAM of your place of residence. You will receive a pre-filled file with your information, all you have to do is add a photo ID, a photocopy of your ID and sign the document: you can now send it back correctly filled in. The health insurance company will send the card to the insured person by mail (to the Centre numérisation photo, 72046 Le Mans cedex 9), within 15 days.
If you wish to have a carte vitale from the age of 12, all you have to do is ask your parents to apply to their CPAM.

Making your first request for a carte vitale on your Ameli account
Even if you don’t have a carte vitale yet, you have a social security number that allows you to create an Ameli account, and from the dematerialized services, you can make your carte vitale request on your Ameli account. We explain how to do it.
In practice, step by step, you must connect to your Ameli account. From the home page, you must go to the tab “My procedures, my health insurance card, order a health insurance card”. When you arrive on the dedicated page, you must select your profile or that of the insured concerned. The CPAM will then remind you of the information you have provided: marital status, postal address, e-mail address.
To validate your order for a carte vitale, documents are required: you will have to upload a scanned passport photo and a scanned identity document. Check that all the information and documents are correct, then validate your order.
That’s it, your request has been transmitted to your CPAM, in complete security. If all your documents are in order, you will receive your carte vitale by mail within 15 days. With this card, the Health Insurance will send you information related to your rights that will be useful if you lose it.
Order your carte vitale by phone or by mail
If you do not have an Ameli account, you can order your carte vitale by other means such as by phone or by mail.
To apply for a carte vitale by phone, there is only one number to remember: 36 46, a service available from Monday to Friday from 8:30 am to 5:30 pm.
If you are in the DOM-TOM, other numbers have been set up. From Reunion Island, dial 0 811 300 974 from Monday to Thursday from 7:30 a.m. to 4:00 p.m. and on Friday from 7:30 a.m. to 12:00 p.m.
In Guadeloupe, dial 0 810 103 646 on Mondays and Thursdays from 7:30 a.m. to 3:30 p.m. and on Tuesdays, Wednesdays and Fridays from 7:30 a.m. to 12:30 p.m. From Martinique, dial 0 820 222 555.
